Keira Stevens is a board certified Psychiatric Nurse Practitioner with over 20 years’ experience in the nursing profession. She has worked with both children and adults in inpatient and outpatient psychiatric settings in Arizona and New Mexico.
Telemedicine is a medical appointment made through video as opposed to an in office appointment. We use a secure patient portal with a video conferencing system. We do not record any sessions, however notes are taken for the medical record. We request that you be in a secure, private place, free of distractions as this is a medical visit and personal information will be discussed. You must be in your home state.
We are currently credentialed in Arizona with most commercial insurances including: United, UMR, Multiplan, Optum, Medica and Oscar, Cigna, Aetna, Magellan, BCBS. Please call for a complete list. We do not take Medicare or Medicaid. We have cash pay options available. We can provide you with a superbill you can submit to your insurance for reimbursement, however we cannot guarantee reimbursement. Your insurance will be verified before every visit. All payments are due at the time of service via a secure credit card portal and card on file.
We are currently credentialed in New Mexico with most commercial insurance plans including: including Cigna, United, UMR, Optum, Multiplan, Molina, Magellan, Western Sky, Aetna, BCBS, Presbyterian, Centennial and Medicaid. Please call for a complete list. We also have cash pay options available for those without insurance. We can provide you with a superbill you can submit to your insurance for reimbursement, however we cannot guarantee reimbursement. Your insurance will be verified before every visit. All payments are due at the time of service via a secure credit card portal and card on file.
Please call our office in order to schedule an initial evaluation. All paperwork and forms are required and must be completed 48 hours prior to appointment confirmation. We use a secure patient portal system for video conferencing. We use a secure financial third party for credit card transactions and require a card on file for any fees or co-pays. Once your appointment has been confirmed you will receive a confirmation via text, email, or phone call based on your preference. Follow up appointments are generally made at each visit.
For established patients we do offer a limited number of same day or next day emergency appointments on a first come first serve basis. These are not double booked or overbooked appointments. Please use the patient portal to request an earlier appointment and we will try to accommodate your request based on availability.
We do not double book or overbook appointments. Your appointment time is dedicated entirely to you. We request that you show up on time in the video conferencing queue. We are aware emergencies happen but we wish to respect each patient’s valuable time. If you need to reschedule your appointment please do so 48 hours in advance. When we schedule your initial
evaluation we require a credit card on file to secure your appointment. This information is held securely by a third party and will be used for any fees or co-pays due at the time of service. All pediatric patients must be accompanied by a legal guardian at each visit.
Cash pay fees (for those who do not have insurance) for both Arizona and New Mexico are as follows. See appropriate state for insurance plans accepted:
Initial Evaluation: $250
Moderate Follow-up: $125
Complex Follow-up $150
Provider phone call between appointments: $15
Paperwork for FMLA/Short Term Disability (per each completion) $50
Initial Evaluation No Show $100
Follow-up No Show $75
Medical Records requests: $25
If medications are recommended, your provider will prescribe psychiatric medications in accordance with best practice. Medication changes require an appointment. Close monitoring of medications is essential to evaluate for response and monitor for side effects. You will receive enough medication refills to get you to your next scheduled appointment. All controlled
substances require an appointment for refills, no exceptions.
We do not provide any evaluations for court or disability. We do not provide child custody evaluations. We do not provide letters for Emotional Support Animals. We will help you with FMLA and Short Term Disability paperwork but cannot guarantee benefits.
